What happened
Employee was reinstalling idler machine. The machine was not running at time of installation. Employee did not lock out tag out machine so power was still active. He went to tighten bolt that holds idler on and the machine started due to sensor reading movement. Finger was caught in drive belt which amputated right pinky distal phalanx.
