The Tube Forming employee was returning to his bend station on the east side of the tube forming oven. He was not carrying a tube forming fixture at the time and his hands were free. A foot allegedly caught on the edge of ergonomic matting. It was reported that the matting was torn curled. He lost his balance and fell. When he fell he struck a tube forming fixture (1042859-1569 X 2) that was leaning against a bend table. His 3rd finger on his right hand was bent backward and he fell on his right shoulder.
