Troubleshooting Pre-Refiners

Employee was asked to assist with the startup of the prerefiner which was not running as intended due to intermittent level transmitter feedback fluctuations (likely due to splatter of mixer mass on level transmitter area). Employee opened prerefiner hopper discharge using manual functionality on the HMI but a much larger-than-expected volume of material discharged from the hopper overflowing the trough and eventually building up on the back side of the machine safety cover. The pressure from this buildup caused the safety cover to give way and rotate away from the machine striking the employee 's right pinky finger causing a modest laceration.
