Before the incident
Employee was working within the water room for roughly an hour [REDACTED] the accident occurred continuously stepping over the hose that he eventually trips on.
What happened
Employee attempted to step off the concrete platform in the water room and over a hose. He caught his left foot in the hose causing him to trip and fall catching himself with his right foot leg arm. Employee refused medical attention at the time then sought attention independently notifying employer after attention was sought.
Injury or illness
Employee underwent toe surgery on his right foot earlier this year (unrelated to work) and is currently under medical supervision to determine any damage done to the surgical site related to this trip and fall incident. Medical treatment was just recently initiated with the use of a bone stimulator machine.
