Before the incident

What happened
The crane operator was holding the container in a floated position above a chassis to allow the dock men to remove the twist locks safely. After removing the front twist locks the employee proceeded to the rear of the container to remove the remaining locks. As this was occurring the landside dock man signaled the truck driver to back the chassis into position to align it with the floated container. While the water side dock man was positioned at the rear and in the process of removing the right-rear twist lock the crane operator unexpectedly lowered the container onto the chassis trapping and injuring the dock man 's right hand.
