What happened
[REDACTED] was swapping a full cullet hopper with an empty one. After putting the cover on the hopper [REDACTED] was guiding [REDACTED] where to set the hopper. Right before [REDACTED] set the hopper down he side-shifted slightly. The hopper was wobbling just a little when it touched the ground the hopper hook released and the hopper tilted forward. When it did the cover rolled off the hopper and struck [REDACTED] as he was trying to get out of the way. We do not think the safety chain was secured in the safe position. [REDACTED] sustained and injury to his left foot. He was taken by first responder to the first aid room via wheelchair and evaluated. [REDACTED] is being taken to Dr. [REDACTED] for further evaluation.
