Before the incident
The teammate was assisting in getting the equipment up and running.
What happened
On [REDACTED] the teammate stepped on a piece of plywood that was covering a 2 ' deep hole that was created from removing equipment out of the [REDACTED]. When the TM stepped on the plywood cover the plywood broke causing the TM to fall into the hole. TM hit his left shin causing a laceration. TM was evaluated by the shift EMT and the on-site occupational health and safety coordinator where he received first aid treatment. TM returned back to work.
Injury or illness
The teammate reported to the company nurse on 05-18-25 that his laceration on his left shin started oozing. On 05-19-25 the company nurse took the teammate to St. [REDACTED] Business Health where he received an antibiotic to treat infection as well as prevent future infection.
