Before the incident
The teammate was pulling up the roll change cylinder hook with a 32” no-touch tool on the [REDACTED] roll change cylinder head to remove it from the draw bar of the dirty set of [REDACTED] rolls. The teammate motioned for another teammate to pull the cylinder back by utilizing controls at the roll change control panel.
What happened
When the cylinder retracted the hook caught the draw bar and pulled the work rolls into the roll change cylinder guarding. The teammate 's right foot was in between the draw bar and the guarding.
