As the teammate was trying to place the east side of the [REDACTED] back in the track the west side of the [REDACTED] came off the track. When the west side of the [REDACTED] came off the track the east side fell to table below while the teammate was still holding on to it.

The teammate received a strained right forearm. This injury was originally a first aid and didn 't become an OSHA recordable until 06 18 25 when the teammate received a steroid prescription.
