Plugging in Scissor Lift

What happened

Initial Report - On [REDACTED] [REDACTED] (IE) was plugging in a power cord to charge a scissor lift. When he stood back up from plugging in the outlet he bumped the back of his head on a ladder rack that was above the cable outlet. This has caused a welt on the back of his head along with slight headache discomfort. I am currently monitoring his state and if headaches or other issues come up I will escalate immediately. [REDACTED] - IE 's head was hurting and he opted to go home before the end of his shift. IE was instructed if he did not want to go to Occ Med then to call MCM. [REDACTED] later UP supervisor was notified IE went to his own doctor and was told he had an acute concussion. [REDACTED] - IE did not come into work today due to headache. [REDACTED] Mgr. was calling Occ Med to get appointment for IE. The physician who performs follow ups on concussions is the same as our Occ Med doctor for the [REDACTED] office so IE was going to follow up on [REDACTED] [REDACTED]. Concussion
