What happened

Approximately 2100 [REDACTED] Operator (IE) was walking through [REDACTED] when he tripped on a group of studs protruding from the ground belonging to a handrail that was removed due to a prior incident (IE-2508-0038). It should be noted that none of the studs were covered from preventing someone from tripping at the tie of injury. IE had tripped on the studs and landed on his knee before continuing to fall forward and hitting his head on a secondary containment pallet. IE got up and notified his supervisor via cell phone. IE 's knee was swollen and had an abrasion located on his scalp and L. knee he attempted to continue working but ultimately had to go home due to swelling and discomfort. [REDACTED] Supervisor notified Safety about the incident. Safety called the plant and spoke with Aseptic supervisor to ensure the studs were covered and the area taped off.
