Before the incident

EE went from the cure room to the bathroom

What happened

EE was in a hurry to get to the bathroom and in her haste she sat abruptly on the toilet which caused an aerosol can that was resting on the back of the toilet to fall. EE though it was her phone and went to try and grab it. As she moved she hit her head on the handicap toilet railing in her upper right side of the head near the hair line. EE lost vision and squeaked her teeth. EE started feeling dizzy on her way to the breakroom. In the breakroom EE informed their supervisor [REDACTED] about the incident. Supervisor advice they would fill out an incident report when the ee returned from lunch. EE went to lunch and did not return to work until [REDACTED]. EE states that toilet area did not have any hazards like slipping or tripping area. On [REDACTED] EE spoke with EHS and Medcore was called ref#2763829 they instructed her to go to the hospital. EE clocked out and headed to the hospital.
