What happened
Contusion swelling bruising to left temple: The employee was clearing a jam in the salwasser. The employee did the correct RRI for the task. Once the employee locked the door back the employee assumed they pushed the flaps arm all the way open. Whenever the employee reach in and turned to come out of the machine their face came into contact with a flap arm that was not opened all the way.
