Descending a ladder while carrying tools

Employee was using an 8ft ladder to clean the fan near the PET grinder. As they were descending the ladder they missed the last rung which resulted in them falling backwards to the floor. Employee sustained a laceration to head wrist and inside their bottom lip. It is not certain if employee hit their head on the crusher or floor at this time. When initially speaking with the employee they were forgetting how the incident happened or what they were doing. An EMS was called for transport to the hospital.
