Employee was operating a forklift inside [REDACTED] Cold Room C at the [REDACTED] removing six pallets from the third rack tier.

On [REDACTED] at approximately [REDACTED] the employee sustained an eye exposure injury when a forklift hydraulic hose ruptured spraying hydraulic oil into his eyes while removing a pallet in Cold Room C. He initially rinsed his eyes and declined medical attention after consulting with his supervisor. During his next shift he reported discomfort in his right eye and was escorted to the eyewash station for a 10-minute rinse and given additional eye solution to take home.
