Before the incident
Employee was operating a forklift inside [REDACTED] at the [REDACTED] facility. He was in the process of picking up a pallet from the first tier.
What happened
While operating his forklift inside [REDACTED] at the [REDACTED] facility EE was picking up a pallet from the first tier. During this process the hydraulic hose on his forklift suddenly ruptured causing hydraulic fluid to spray directly into his eyes. The rupture occurred after he had just finished lowering 14 pallets of romaine from the third tier which could have placed additional strain on the hydraulic system. Immediately following the incident EE was taken to the nearest eyewash station. He proceeded to rinse his eyes for approximately 10-15 minutes. He was also provided with an eye solution to help alleviate the irritation. Despite these first aid measures [REDACTED] requested medical attention for further evaluation and was sent home pending authorization for a clinic visit the [REDACTED]
