What happened
He pulled out of the trailer on his electric pallet jack. When he was turning the pallet jack to turn around and get another pallet he did not see another employee on a forklift nearby. The pallet jack hit the forklift. His hand holding on to the throttle was the first thing that hit the forklift and his hand got pinned crushed between the vehicles.
