Adjusting the bags running through her machine. She was attempting to remove a jammed bag with the help of another employee.

The employee that was assisting her with removing the jam pressed the button to cycle the machine without verifying that the injured employee was ready for her to do so. This triggered the jaws of the machine to close when her hand was between the jaws.

The tip of her right index finger was cut by the jaws. It removed 2-3mm of skin and part of the nail. It was classified as an avulsion because it just removed the skin from her finger.
