Adjusting armrest on a wheelchair.

On [REDACTED] [REDACTED] [REDACTED] bent over to adjust the arms of a wheel chair that was going to be used for a patient. When he bent over he felt and heard a pop in his back. He went to stand up and he couldn 't stand up straight and fell to his knees. He was in pain and laid flat on the floor. [REDACTED] came and got myself [REDACTED] (Branch Supervisor). When I went into the warehouse [REDACTED] was standing at the workbench and could not put his foot on the floor. He walked with assistance to my car and I took him to Urgent care. I had use a wheel chair to get him inside the clinic. He checked in at [REDACTED] and then I left him with the providers. [REDACTED] also called Medcor and is also bringing the paperwork from the provider. I have attached pictures to this report as well.
