What happened
On [REDACTED] at approximately [REDACTED] a forklift driver was moving a metal tool about 15 ft. long with the forklift. He used the forklift to maneuver the piece into place to put it in the back of a box truck to transport to another location. As he was getting the tool lined up he used the fork to maneuver it into place as he did this the EE was standing at a work bench with her back to the forklift. He swung it into place he went too far and pinned EE Left calf between the bench and the tool. he had to get off the forklift to remove the tool. the injury was not reported to the safety department until [REDACTED] EE did not want to the Dr at this point. she later changed her mind and was taken into Concentra on [REDACTED] at [REDACTED]. she had bruising to the back of her calf and will recover.
