Before the incident
On [REDACTED] employee pulled up the dock plate at door #30 and chained it off the preceded to grab the cord to close the trailer door. When he grabbed the trailer cord the dock plate chain dislodge and the dock plate came down about 9 inches and fell on the employees right leg (shin) just below the knee cap. Employee came to the office and first aid was administered and the employee refused medical attention. On [REDACTED] employee called the supervisor stating he was calling out because he was still bleeding. Supervisor notified ESS Manager and at this point employee was sent to Concentra to have his right leg looked at. Concentra treated the employee for a laceration which required eight sutures.
