Before the incident
Employee noticed that the line for the glycol mixer on bread line 1 was frosting. He went to check the sponge mixer panel and the panel showed the pump was off. When employee felt the line it felt like the it was still running. Employee then went to notify supervisor and they both checked the computer in the Engineering office and it also showed that the pump was off. Employee then left to back to the bread line 1 where sanitation was still cleaning and asked when they would be done so he could do his PMs. He was told 30 minutes so at this time the employee went to the compressor room to check the pump and noticed it was running. Employee checked the sponge mixer panel again and it showed off. At this point he went up to the chill water mezzanine to check the contactor and overload that controls the pump. Contactor was stuck closed and when he used his meter read to check the contactor the overload exploded. Employee was off to the left of the panel when checking and the arc flash went
