Before the incident
Employee had stepped up from the press beam onto the belt. They crawled to the back hem and pulled it to align it correctly on with the buffer and belt edge. The got back into a crawl position and stepped the left foot backwards off the belt and onto the press beam

The left foot slipped on the beam and the employee ended up falling off the belt and into the area between the stretcher and press on the ground.
