The employee had crouched down and grabbed the end of the belt. The belt was on pallet on other belts. They stood up and started pulling the belt towards themselves and walking backwards. The back v-guide got caught on the belt underneath. The coworker reach and grabbed to back of the belt and lifted up and back to give it some oomph.

When the oomph happened the employee jolted back and the foot tripped on a belt on the floor. The employee backwards and hit their head and left leg on the ground.
