helping patient to bathroom and back

[REDACTED] was walking patient to bathroom with gait belt on. When they made it to the toilet he lost balance and fell towards the wall. [REDACTED] kept holding on and called for nurse. They were able to get the patient seated on the toilet. He went to the bathroom. [REDACTED] thinks he lost consciousness. He denied any LOC dizziness lightheadedness etc. and when [REDACTED] asked him if he could make it back to bed he said yes and they headed back to bed. Once they made it to the bed [REDACTED] asked the patient to turn around and she says he lost consciousness again. She was talking to him and he was not answering but eyes were open. She called for nurse again while she was trying to hold him up with both hands so he wouldn 't fall. This is when she heard and felt her left finger pop. She returned to work the next day but went to urgent care [REDACTED] when the pain was not subsiding.
