What happened
[REDACTED] was working in the dish room pulling dishes from the dish machine. When a large container got stuck inside the machine when he went inside to fix the container another pallet came from behind it and slammed the container to the roof. Jamming his hand to the top where a fan was on and caused it to take a chunk from the top of his thumb near his fingernail. The machine was then stopped and his hand was freed from the racks and equipment.
