What happened
I am filling this out on behalf of [REDACTED] who reported this workplace injury to me.Flu shot was administered after caregiver 's shift on [REDACTED]. Caregiver writes It didn 't hurt going in but there was an intense pain as it was being injected. I can still feel where it was given and it was too high. I have had pain ever since. I expected it to heal and didn 't worry too much until I realized [REDACTED] that it was getting worse not better. I have constant dull pain in my shoulder that is bearable. Certain movements cause intense pain. I have limited movement the pain radiates down my arm and I have periods of numbness tingling in my pointer and middle fingers. I called in sick and went to my doctor [REDACTED]. He prescribed prednisone and tylenol 3 and PT with ultrasound therapy.
