Charting

Patient confused and requiring very physical assistance when providing cares. At times more agitated and resistant to repositioning. He often would be positioned with head down between legs so far he would need physcial propping to not hard himself. Frequently stiff pushing back against repositioning. During the night I was also assaulted (see seperate report) between this patient and the assault my right shoulder and upper thoracic back has sore with some nerve pain with shooting down my right arm. Overall a difficult situation with patient mentation agitation posturing. Mobility equipment would have not helped or been appropriate for situation. Patient often on all fours and scootching self down bed or in various other unsafe arrangements in room. Son at bedside at all times bed alarm in place.
