What happened
On [REDACTED] the bottom floor draw operator set up the winder to begin spooling fiber. The initial fiber was manually routed by the operator onto the pulleys. Once routing was complete the operator used a foot pedal to begin rotating the spool at a low rate of speed to start the initial spooling process. Once the initial spool was complete the operator closed the door and switched
