What happened
employee bend to picked up article from floor near the garbage container in [REDACTED]. She stated she raised up her head and hit her head it at the edge of the metal holder of the wipe container. She walked out of room and saw her preceptor [REDACTED] and informed her of the incident. She called the charge nurse to report incident and went back into [REDACTED] to do VS for patient. Charge nurse met her in room and SAFR was done. She stated [REDACTED] RT witnessed the incident. [REDACTED] stated she saw employee hit head from the corner of her eye. She stated she heard a thump. She stated employee said you did not see that Miss [REDACTED] did you? [REDACTED] replied Yes I did. Employee states she will go to urgent care for further evaluation.
