Before the incident
A forming tool was not installed in the 42 inch hydroformer press properly The operator stopped work and asked a more tenured operator to fix the issue As the more experienced operator tried to resolve the issue he drove some of the tooling upward with no where to go because it was under other tooling pieces This caused a holddown clamp (aka QDC) to break This resulted in a steel locating pin breaking and was liberated from the press A portion of the pin made contact with the less tenured operators right ear and right side of head
