Before the incident
The employee was offloading an empty 10 x 3 pallet from a forklift (see Photo 1 attached) After positioning the forklift the right side of the pallet was pushed onto a stack of empty pallets Subsequently the left side was pushed and pulled to adjust the pallets position During this process the edge of the pallet slipped off the forklift resulting in the employees arm and shoulder being pulled
