Before the incident

Operator was standing and a cutter changeout station with an endmill vertically mounted in the TorqueFit fixture The cutters sharp end was standing in the air and ready to have its collar loosened so the operator could remove the cutter The operator did not have the protective sleeve over the sharp end of the cutter The person reached towards the bins that were mounted on the back and to left side of workbench to retrieve a sharp replacement cutter In doing so the individual reached over the exposed cutter and lacerated the underside of his right forearm approximately half way between the hand and the elblow
