What happened

On [REDACTED] an employee reported feeling pain in the head after being struck by a falling case. The employee was unloading (DD38) with another employee when a case weighing approximately 40 pounds fell striking the employee on the top of the head. During the investigation it was identified that moments prior to being struck on the head the employee was struck in the forearm by a falling case. When speaking with the witness they demonstrated the unload process with 2 employees inside the trailer. The flex line was pulled into the trailer (centered) approximately 3 feet away from the wall of cases. Employees were unloading the trailer from each side (top to bottom) before moving onto the middle column that remains. This results in unstable loads. When asked if they were trained to unload this way the witness responded to be completely honest I was never really trained on unloading inside the trailers.
