placing vacuum lift suction cups on box

On [REDACTED] at approximately [REDACTED] a Wayfair associate working in bulk picking reported PAIN LEVEL 9 pain in their left eye due to suction cup coming off striking associate on the left eye. The associate was working at T-3 and stated During placing suction cups on a box and the suction cup came off hitting associate on the left eye. During the investigation the site found that the associate was hired on [REDACTED] completed bulk picking training on [REDACTED] and IS currently up-to-date on Bulk Picking training. The associate WAS NOT working in a process path that requires job rotation. The Associate has been working in Bulk Picking for approximately 9 months. The investigation;identified the associate WAS wearing their PPE at the time of the incident and WAS found to be following Wayfair safety policy procedure.
