The employee tripped over a pallet fell with his left arm and left hand causing discomfort to his left hand Newly reported symptoms to lower back right knee and left ankle during 24 hour follow up on [REDACTED] On [REDACTED] the MidWest Fleet experienced an injury that has progressed from a firstaid only case to a Recordable Injury A driver (DOH 3 27 23) was delivering to a SherwinWilliams store in [REDACTED] The driver stepped over a pallet containing one small box in order to get to the light switch to shut off the stockroom lights In doing so he tripped and fell with his arms outstretched He broke his fall with his hands elbows and right knee Axiom was contacted and firstaid measures were followed by the employee Axiom continued to follow up with the employee When the employee reported improvement Axiom closed the case on [REDACTED] On [REDACTED] the employee reported worsening in his left hand Axioms case was reopened and the employee was ultimately seen at a local urgent care faci
