[REDACTED] appeared to have reached into the lead-out side of Press 64 while Operator [REDACTED] jogged Press 64 under the direction of Pit Crew EE [REDACTED]. Operator [REDACTED] manually jogged the press 5-[REDACTED]0 times without steel in order for Pit Crew EE [REDACTED] to visually inspect the rollers on the feed (feed is on the lead-in side of the press). Operator [REDACTED] jogged Press 64 one more time and heard a scream and stopped jogging the press. Operator 3 working on Press 66 notified Operator [REDACTED] that [REDACTED] was injured. [REDACTED] appeared to have amputated 4 fingers on his right hand during the last actuation of the press.
