A thin spot on the sheet resulted in the sheet coming off the chain rails and jamming up in the former. The employee shut off machine and locked it out. The employee used a Level 4 knife to slit the sheet so that he could insert his j-hook knife. The employee states the knife slipped and he cut his non-cutting arm with the exposed blade Level 4 knife. The employee states he wore the correct gloves but failed to wear the required cut protection sleeves.
