Before the incident

EE was working at packing end of [REDACTED] [REDACTED] lifted a box off of the floor that was approx 30lbs to put on conveyor Due to the cases being made wrong the EE had to flip the case over once got close to the conveyor Prior to EE putting on conveyor EE hit right arm on metal plate causing them to go down to the floor EE was unable to stand back up due to sharp pain in right hip area EE received first aid from AT for about 2 hours and still could not bear weight on right leg or walk without heavy assistance EE was taken to emergency room for evaluation Diagnosed with muscle strain
