Employee was working in Warehouse 1.

The injured employee was working in [REDACTED] 1 when another forklift driver came in to retrieve some materials from one of the racks. There were two pallets of stacked cartons that the driver needed to move to access the rack of materials. Upon attempting to move the pallets the some of the cartons on the bottom pallet started to fail causing the top pallet to shift. The affected employee came over to try and help the situation and grabbed the top pallet attempting to help steady it and help get it into a more stable position. The forklift driver had positioned the forks inside of the top pallet but the affected employee did not realize they were there. The driver shifted the forks to the side attempting to steady and remove the pallet upon which point the affected employee 's left middle and ring fingers became pinched between the forks and the pallet.
