What happened
On [REDACTED] while [REDACTED] was organizing bags and boxes inside the delivery van one of the metal shelves inside the van was inadvertently lifted up. The shelf then suddenly dropped down and struck [REDACTED] in the back approximately at the waist area. At the time of the incident [REDACTED] was standing inside the rear compartment of the van arranging delivery items. Immediately after the shelf came down [REDACTED] felt a sharp impact and discomfort in the lower back waist region.
