Before the incident
On [REDACTED] at [REDACTED] EE left a voicemail with his supervisor to report he was calling in sick due to an injury he sustained at work the [REDACTED]. EE stated he was loading the UPS hand stack trailer last night and rolled his ankle when he stepped out of the operators platform. EE didnt think it was serious so he didnt report it to his supervisor. EE stated the platform was at its lowest setting when he stepped to the floor. Two things I noted when doing the investigation were the lack of handle to grab onto to enter and exit the platform and the stability of the platform especially when fully extended. When fully extended the platform bounces up and down when you stand on it or move around. This can make exiting the platform even more difficult especially with no handle to hold on to. The details of this investigation report were gathered over the phone. More details may be available when I can meet with the operator in person.
