Before the incident
On [REDACTED] at [REDACTED] EE came to the supervisor 's office to report an injury. EE stated they experienced back pain while wrapping their pallet. EE had just applied the paperwork to a [REDACTED] pallet that was 26.5 tall. Due to the pallet height the EE bent over to apply the pallet wrap. EE stated while they were bent over they twisted their torso as the wrap was applied to all 4 sides of the pallet. The EE experienced back pain after making one full wrap of the pallet. The pain caused them to go to their knees and use their left arm to stabilize themselves as they went to the ground. EE was asked if there was a wrap stick present at the print station but they did not know if there was. The reenactment photos were taken at the print station the injury occurred at. There was a wrap stick present at the print station. EE was seen by ERT and the OHN. EE was assigned duties that avoided bending over and twisting their body.
