What happened
[REDACTED] was pulling a ladder for an order when he pulled the ladder out another ladder fell on and hit him. [REDACTED] had a cut on his head and forehead. He was taken to Concentra and received stitches and was determined not to have a concussion. The associate went home after he left Concentra. I do not have a statement from him or a doctor 's note since he left immediately after he was released. I plan on adding any additional information I can get including doctor 's notes and a statement from [REDACTED] as soon as I get in contact with him [REDACTED].
