EE was responding to a winder break on [REDACTED] and was attempting to apply tape to a partial roll while under power to recover from the winder break The tape did not adhere to the foam and as the employee tried to reapply the tape they lost control of the core of tape due to the rolls continued rotation This caused the core of tape to complete its rotation swing around and graze the employee above their upper lip and below their nostril
