What happened
On [REDACTED] the injured person (IP) was working along with two other coworkers unloading equipment off of trucks. At approximately 15:20 [REDACTED]. while lifting item 4690 the IP working as a spotter went around the material to the “blind side” to check and see if the forks had penetrated correctly. The IP did not communicate his intentions to the signalmen or the operator of the forklift. The IP stated he had cut the blue shrink wrap to get a visual on the fork position. As he folded the plastic his left index finger was caught between the left fork and the equipment amputating the tip.
