EMPLOYEE WORKING THE TRUCK DOCK AS AN OFFICE AGENT WAS STANDING NEAR DOCK DOOR AS AN IMPORT TRUCK BACKED INTO DOOR 5. THE DRIVER BACKED THE TRUCK INTO DOOR 5 AND APPROXIMATELY [REDACTED] LATER PULLED THE TRUCK AWAY FROM THE DOCK AS HE FORGOT TO ADJUST HIS TANDEM AXLES TO THE REARMOST POSITION. DURING THIS TIME THE EMPLOYEE DID NOT ENGAGE THE DOCK LOCK SYSTEM AND INSTEAD PUT THE DOCK PLATE DOWN AND STEPPED ONTO THE TRUCK. WHEN THE TRUCK PULLED AWAY THE EMPLOYEE JUMPED TO THE GROUND AND LANDED ON HER FEET AND WALKED AWAY FROM THE DOCK AND SPOKE TO THE DRIVER. THE EMPLOYEE INFORMED THE ACTING OSM ON DUTY AND CALLED THE STATION MANAGER. THE STATION MANAGER SPOKE TO THE AGENT DIRECTLY AND THE EMPLOYEE REFUSED ANY TREATMENT AND WAS JUST SHAKEN UP
