EE was unloading cylinders from outside hauler

EE was unloading cylinders from outside hauler when back out with cylinder on the forks the right side contacted a cylinder crate causing it to fall off the truck and out of the crate. EE and outside driver were teamlifting the cylinder back into a crate it fell out of. The driver lost his grip on the cylinder causing it to fall and EE finger was pinned in between cylinder and crate.
