What happened
The GMM was using a socket wrench to tighten a flange bolt on a butterfly valve in the [REDACTED] 5 pump room. The socket wrench with cheater pipe slipped off of the bolt head which caused him to lunge forward. He struck his face on the valve actuator which cause a laceration directly below his left eye
