It was observed that there was powder leaking from the access door for the #10 Electrolytic iron ball mill which is a rotating mill containing steel balls from grinding metal powder. The mill was stopped and locked out to investigate. It was determined that the lid needed to be removed and a new gasket installed to fix the leak. The employee proceeded to the mill to remove the door.

Due to the fact that the mill lid was hard to remove the employee hammered steels wedges into the gap between the lid and the mill body to attempt to break the lid free so the gasket could be replaced. As he was hammering he felt a stinging sensation in his right wrist.

Torn ligaments in right wrist.

The hammer and the steel wedge being used in combination to try and remove the lid from the ball mill caused this accident to occur.

Tron ligaments in right wrist. While trying to disengage the lid to a ball mill with a wedge and sledgehammer employee felt pain in wrist.
